You'll conduct assessments, develop tailored plans, and collaborate with government bodies and service providers to improve welfare outcomes for families, individuals, and at-risk groups.

Qualifications

• Minimum 1 year full-time relevant experience (disability support, community services, or allied health assistance).
• Relevant qualification (e.g., Certificate IV/Diploma in Community Services, Disability, or Mental Health).
• Strong interpersonal and communication skills for liaison with diverse stakeholders.
• Ability to work independently and collaboratively within a multidisciplinary team.
• Current driver's license and willingness to travel within the community.

Responsibilities

• Conduct in-depth client assessments to identify needs, then design and implement tailored educational, training, and support programs that address social, emotional, and financial challenges.
• Interview clients to evaluate the nature and extent of their difficulties, and develop individualized case plans with clear, measurable goals.
• Monitor client progress through regular check-ins, document outcomes, and prepare detailed reports for internal review and external agencies.
• Refer clients to appropriate services—such as housing, employment, or health providers—and coordinate multi-agency support to ensure holistic care.
• Assess community needs and resources related to welfare, housing, employment, training, and other facilities, then advocate for improved services.
• Liaise with community groups, welfare agencies, government bodies, and private businesses to address local issues and promote awareness of available resources.
• Support families and care for children, disabled persons, or elderly clients in group housing, adult service units, and institutional settings.
• Supervise offenders on probation or parole, ensuring compliance with court orders and facilitating rehabilitation programs.
• Assist young people in resolving social, emotional, and financial difficulties through counselling and practical guidance.
• Prepare funding submissions and detailed reports for government bodies and other agencies to secure resources for community initiatives.
