Job summary

Plena Healthcare invites a Physiotherapist to join our national allied health team delivering personalised care in residential and community settings across Victoria. You will work within a large multidisciplinary group to assess, treat, and support mobility, independence, and daily living goals.

• General AHPRA registration is required.
• Current Police Check or willingness to obtain.
• Current driver’s licence and access to a vehicle.

• NDIS Worker Screening Check or willingness to obtain.
• Full Australian working rights.

Responsibilities

• Provide comprehensive assessments and clinical recommendations to support daily living, mobility, transfers, falls risk and prevention, equipment needs, pressure area care and skin integrity.
• Manage a wide range of acute and chronic movement-related conditions in community settings.
• Deliver mobility and functional programs, care plan reviews, one-to-one interventions and group-based active ageing sessions.
• Collaborate with a multidisciplinary team to provide personalised, evidence-based therapy programs.

As a qualified Physiotherapist working in a community setting,you will be joining a large and collaborative multidisciplinary team providing personalised treatments.

In this role you will be managing a wide range of acute and chronic conditions which affect movement, function, and independence.

• The clients you will seeing in the community are referred through the following government funded models, Home Care Packages, Short Term Restorative Care, Commonwealth Home Support Programme and Private.
• Provide comprehensive assessments and clinical recommendations to support consumer activities of daily living, mobility, and transfers, falls risk and prevention, equipment requirements, pressure area care and skin integrity management, arthritic joint management, and pain management.
• Demonstrate flexibility and adaptability to the needs of Plena and community-based providers to work within the scope of the Physiotherapy profession.
