Job summary

Plena Healthcare is seeking a Physiotherapist to deliver evidence-based therapy programs in Residential Aged Care, working with a multidisciplinary team. The role includes mobility and functional programs, care plan reviews, one-to-one interventions, and group-based active ageing services across RAC and community settings.

You will collaborate with colleagues, residents and families, hold General AHPRA registration, and meet checks and driving requirements.

Qualifications

• General AHPRA registration is required.
• Current Police Check or willingness to obtain.
• Current driver’s licence and access to a vehicle.
• NDIS Worker Screening Check or willingness to obtain.

Responsibilities

• Deliver mobility and functional therapy programs.
• Conduct care plan reviews and adjust interventions.
• Provide one-to-one patient interventions.
• Facilitate group-based active ageing services.

This Physiotherapist role focuses on Residential Aged Care, working alongside a multidisciplinary team to deliver personalised, evidence-based therapy programs, including mobility and functional programs, care plan reviews, one-to-one interventions, and group-based active ageing services.
