Job summary

Team Rehab Solutions in South Brisbane is seeking a passionate Community Occupational Therapist to join our growing multidisciplinary team. You’ll work in the community, helping clients improve independence, function and quality of life in their homes and communities.

We offer flexible hours (full-time or part-time, Monday–Friday), a company vehicle and laptop, a $2,000 CPD allowance, mentoring, and staff and family discounts.

Qualifications

• Bachelor’s or master’s degree in occupational therapy.
• Current AHPRA registration and professional indemnity insurance.
• Minimum 1–2 years’ experience preferred.
• Community or NDIS experience highly regarded (but not essential).
• Strong communication skills and a genuine people-focused approach.
• Self-motivation, initiative and the ability to work collaboratively in a team.
• Blue Card (Working with Children) and NDIS Worker Screening Check (eligible to obtain).

Responsibilities

• Functional assessments and ADL support.
• Capacity building programs in home and community settings.
• Home modifications and assistive technology prescription.
• Neurorehabilitation (stroke, ABI, cerebral palsy, progressive conditions).
• Care coordination and client advocacy across NDIS, Aged Care, DVA and other funding streams.

What You’ll Be Doing

As a Community OT, you’ll help clients improve independence, function, and quality of life within their own homes and communities.

Your Caseload Will Be Varied And May Include

• Functional assessments and ADL support
• Capacity building programs in home and community settings

• NDIS
• Support at Home (formerly HCP)
• DVA
• Private Health
• Workcover

About You

You may be early in your OT career or looking to take the next step. Either way, you’re passionate about community-based care and making a real impact.
