Job summary

Team Rehab Solutions is seeking a passionate Community Occupational Therapist to join our growing multidisciplinary team in South Brisbane (Logan). You’ll deliver care in the community, with a supportive environment and flexible hours.

You’ll work on diverse caseloads, including home modifications, neurorehabilitation, and coordination across NDIS and other funding streams, while collaborating with a dedicated team and enjoying CPD opportunities.

Qualifications

• Bachelor’s or master's degree in occupational therapy.
• Current AHPRA registration and professional indemnity insurance.
• Minimum 1–2 years’ experience preferred.
• Community or NDIS experience highly regarded (but not essential).
• Strong communication skills and a genuine people-focused approach.
• Self-motivation, initiative, and the ability to work collaboratively in a team.

Responsibilities

• Deliver community OT services in clients’ homes and communities.
• Conduct functional assessments and ADL support.
• Develop capacity-building programs in home and community settings.
• Home modifications and assistive technology prescription.
• Care coordination across NDIS, Aged Care, DVA and other funding streams.

What You’ll Be Doing

As a Community OT, you’ll help clients improve independence, function, and quality of life within their own homes and communities.

Your Caseload Will Be Varied And May Include

• Functional assessments and ADL support
• Capacity building programs in home and community settings
• Home modifications and assistive technology prescription
• Neurorehabilitation (stroke, ABI, cerebral palsy, progressive conditions)
• Care coordination and client advocacy across NDIS, Aged Care, DVA, and other funding streams

You’ll work across a diverse community caseload including:

• NDIS
• Support at Home (formerly HCP)
• DVA
• Private Health
• Workcover

About You

You may be early in your OT career or looking to take the next step. Either way, you’re passionate about community-based care and making a real impact.

We’re Looking For Someone Who Has
