Job summary

Bolton Clarke Group in Brisbane North is seeking an experienced Occupational Therapist to deliver home-based therapy within a multidisciplinary team. You will assess clients in their homes, propose options within funding guidelines, and tailor interventions to support functional independence.

Based at Pinjarra Hills and serving Brisbane North suburbs, the role emphasizes consumer-centred care, confidentiality, and collaboration with local health services to improve client outcomes.

Qualifications

• Must be registered with AHPRA and able to practice as an Occupational Therapist.
• Ability to work within primary health care context and funding guidelines.
• Strong communication and culturally safe practice with diverse client groups.

Responsibilities

• Provide high-quality occupational therapy assessments and interventions.
• Coordinate with multidisciplinary team and other providers.
• Deliver home-based assessments covering activities of daily living, mobility and home safety.
• Maintain accurate, timely client records and ensure confidentiality.
• Deliver education sessions and participate in quality improvement activities.
• Engage with community teams and stakeholders to monitor progress.
