Job summary

Care Connector (Holistic Nursing Solutions) is seeking Registered Nurses (RN) and Enrolled Nurses (EN) for community-based opportunities across Brisbane. You will deliver safe, person-centred care in participants’ homes and community settings, with support from experienced clinicians and opportunities to build long-term professional relationships.

The role focuses on complex cases, including spinal injury care, wound care, and continence management, with training and mentoring provided.

Qualifications

• Current AHPRA registration (RN/EN) is required.
• Experience in community or aged care is advantageous.
• Ability to travel to participants’ homes within Brisbane.

Responsibilities

• Deliver safe, person-centred care to participants with complex needs in their homes and community settings.
• Provide high‑intensity clinical care with supervision, including wound care and continence support.
• Contribute to care planning and clinical oversight (RN).
• Guide and support support staff where required (RN).
• Follow care plans and positive behaviour support strategies.
• Monitor and report changes in participant condition; complete documentation.

Registered Nurse / Enrolled Nurse – Remote

Care Connector (Holistic Nursing Solutions) is a clinician-led provider delivering holistic, person-centred care across the community.

About the Role

We are seeking Registered Nurses (RN) and Enrolled Nurses (EN) for community-based opportunities across Brisbane.

This is not high-volume agency work, you will:

• Work with complex and meaningful cases
• Be supported by experienced clinicians
• Deliver care in participants’ homes and community settings
• Build long-term professional relationships with participants

Key Responsibilities

• Deliver safe, person-centred care to participants with complex needs
• Support high-intensity clinical care (with training and supervision), including: spinal injury care, wound care, continence support and catheter care, medication assistance
• Contribute to care planning and clinical oversight (RN)
• Support and guide support staff where required (RN)
• Follow care plans and positive behaviour support strategies
• Monitor and report changes in participant condition
• Complete documentation (progress notes, incident reports)
• Build strong, professional and respectful relationships with clients

What We’re Looking For

• Current AHPRA registration
• A current driver’s licence and reliable vehicle
• Current National Police Check, or willingness to obtain one
• Must have work rights in Australia
• Previous experience in community or aged care highly regarded
• Experience in complex wound care and continence care highly regarded
• Experience with spinal cord injury care highly regarded
• A genuine passion for client-centered care and service excellence
• Excellent clinical judgement and attention to detail
• Strong communication and a calm, professional approach
• Flexibility to travel within the local area to support clients in their homes
• Availability to start immediately is highly regarded
