Job summary

Murrumbidgee Local Health District in Boorowa seeks a dedicated Community Nurse to deliver holistic, person-centred nursing care in community settings. You will collaborate with patients, families and multidisciplinary teams to support chronic disease management and post-hospital recovery, promoting independence and wellbeing at home.

Role is permanent part-time (up to 32 hours/week) with AHPRA-registered nurses; opportunities for professional development and flexible work arrangements may apply.

Qualifications

• Current AHPRA registration as a Registered Nurse.
• Strong clinical assessment, care planning and care coordination skills in community/rural settings.
• Excellent communication and interpersonal skills; ability to work with patients, families and multidisciplinary teams.
• Proactive and adaptable approach; commitment to evidence-based, safe care.

Responsibilities

• Provide holistic nursing assessment, care planning, treatment, education and evaluation of patient outcomes.
• Deliver evidence-based clinical care, including chronic disease management, wound care, palliative care and health promotion.
• Coordinate care with GPs, carers and multidisciplinary teams to achieve optimal patient outcomes.
• Empower consumers and families through shared decision-making, care planning and self-management support.
• Maintain accurate documentation and contribute to quality improvement activities and safe clinical practice.
