Job summary

NSW Health is seeking a Community Nurse to deliver person-centred nursing care in Boorowa and surrounding communities. You will work with patients, families and multidisciplinary teams to support chronic disease management, recovery and independence, helping clients remain safely at home.

Accountabilities include holistic nursing assessments, care planning and coordination with GPs, carers and other professionals.

Qualifications

• Current AHPRA registration as a Registered Nurse.
• Experience in community or rural healthcare settings.
• Strong clinical assessment and care coordination skills.
• Excellent communication and interpersonal skills.

Responsibilities

• Provide holistic nursing assessment, care planning, treatment and evaluation.
• Deliver chronic disease management, wound care, palliative and health promotion.
• Coordinate care with GPs, carers and multidisciplinary teams.
• Support patients to remain well at home and promote independence.
