ACH Group is seeking passionate Registered Nurses to join Community Nursing Teams in SA. You will build relationships, involve customers in care decisions, and empower them to manage their health journey within their homes.

Strong clinical decisions, collaboration, and ongoing development are encouraged in a supportive network. You will conduct assessments, plan care, provide wound and medication management, and document care.

Competenze

• Proven experience as an RN, preferably in community settings.
• Excellent attention to detail and communication.
• Ability to work autonomously with strong problem-solving skills.

Mansioni

• Conduct assessments, reviews, and coordinate care plans.
• Provide nursing interventions including wound care and medication management.
• Support wellbeing of customers living in their own home.
• Observe wellbeing and respond to clinical changes.
• Document care accurately and promptly.
• Participate in on-call after-hours service.
• Liaise with family, Support Workers and Allied Health Professionals.
