Job summary

You will deliver community access support, from outings to daily appointments, and support clients at home while promoting safety and dignity.

Qualifications

• Diploma of Community Services or equivalent is required.
• First Aid & CPR required.
• NDIS Worker Screening Check required.
• Working with Children Check required.
• NSW Police Check or National Police Check required.
• Bachelor's degree in community services, education, sociology, etc.
• 3 years relevant experience or willing to learn.

Responsibilities

• Deliver community access support—from outings to daily appointments or home support.
• Help clients build confidence navigating public spaces and social settings.
• Encourage life skills development, goal setting, and a positive daily routine.
• Provide emotional support, companionship, and transport.
• Assess clients' needs and plan, develop and implement educational, training and support programs.
• Identify challenges and tailor support to meet individual needs.
• Engage with families, welfare agencies and organisations to address community issues.
• Provide counselling and emotional support to help clients navigate personal difficulties.
• Prepare submissions for funding and resources, and reports for government bodies.
