Uniting AgeWell in Victoria is seeking a Community Enrolled Nurse to support our Home Care program from the South Melbourne/Berwick area. The role involves in-home assessments, clinical advice, and collaboration with RN and allied health teams to deliver quality client care.

The position is permanent part-time, three days a week during business hours, with travel to clients. A Diploma of Nursing and ongoing AHPRA Div 2 registration are essential, along with vehicle ownership for client visits.

Qualifications

• Diploma of Nursing with current AHPRA Div 2 registration is required.
• Minimum 3 years as an Enrolled Nurse in Community/Home Care is desirable.
• Proven ability to visit clients in their homes and implement care plans.

Responsabilités

About the role:

Supporting our Home Care program based out of our South Melbourne Metro Home Care office located in Berwick, the Community Enrolled Nurse services Uniting AgeWell clients in the surrounding suburbs of South Melbourne Metro including Cranbourne, Dandenong, Frankston, the Mornington Peninsula and surrounds.

• In-home clinical assessments, reviews and care needs for clients
• With support from the Community RN, provide clinical advice and support to clients and liaise with services for referrals where more specialized support/advice may be required
• Work closely with local allied health professionals to facilitate responsive care in our clients homes
• Permanent part time opportunity, 3 days a week during business hours (may increase is demand grows)
• Balance of in office and home visits depending on client and organisation needs

About you:

This is the place for you if you have:

• Diploma of Nursing or equivalent qualification with current AHPRA Div 2 Nursing registration (required)
• Minimum 3 years experience as an Enrolled Nurse in Community, Commonwealth Home Support or District Nursing (desirable)
• Team player, flexible in your approach to work with a positive outlook
• Experienced in actively visiting clients in their homes
• Experienced with evaluating and implementing care support plans
• Proficiency in decision making, critical analysis and on the spot problem solving
• Demonstrated ability to adhere to professional boundaries at all times
• Proficient at managing own scope of practice, reporting to and under the supervision of the Registered Nurse
• Good communication, problem solving, influencing and negotiation skills
• Own your own registered and insured vehicle (travel reimbursement between clients is provided)
• Thrive in a team and also appreciate the one to one interactions

Essentials for working in Aged Care:

• A current Working With Children Check card
• Current and ongoing flu vaccinations
• Australian working rights
