Job summary

Uniting AgeWell Limited in Melbourne is seeking a Community Enrolled Nurse on a permanent part-time basis. The role conducts in-home assessments and supports clients with care plans within the West Melbourne Metro Home Care network.

You'll work three days a week, with a mix of in-office and home visits, and will coordinate with the Community RN and allied health colleagues to ensure timely, person-centred care for older adults in the community.

Qualifications

• Diploma of Nursing or equivalent qualification with current AHPRA Div 2 Nursing registration.
• Minimum 3 years experience as an Enrolled Nurse in Community, Commonwealth Home Support or District Nursing (desirable).
• Team player with a positive outlook and flexibility to work across home visits and office settings.
• Actively visiting clients in their homes and adhering to professional boundaries.
• Evaluating and implementing care support plans.
• Proficient in decision making, critical analysis and on-the-spot problem solving.
• Own a registered and insured vehicle; travel reimbursement provided.
• Thrive in a team and in one-to-one interactions.

About the role:

Supporting our Home Care program based out of our West Melbourne Metro Home Care office located in Braybrook, the Community Enrolled Nurse services Uniting AgeWell clients in the surrounding suburbs of West Melbourne Metro including Braybrook, Belmont, Bundoora and surrounds.

• In-home clinical assessments, reviews and care needs for clients
• With support from the Community RN, provide clinical advice and support to clients and liaise with services for referrals where more specialized support/advice may be required
• Work closely with local allied health professionals to facilitate responsive care in our clients homes
• Permanent part time opportunity, 3 days a week during business hours (may increase is demand grows)
• Balance of in office and home visits depending on client and organisation needs

About you:

This is the place for you if you have:

• Diploma of Nursing or equivalent qualification with current AHPRA Div 2 Nursing registration (required)
• Minimum 3 years experience as an Enrolled Nurse in Community, Commonwealth Home Support or District Nursing (desirable)

Essentials for working in Aged Care:

• A current Working With Children Check card
• Current and ongoing flu vaccinations
