Job summary

Bayside Health is seeking a Community Care Social Worker to deliver psychosocial assessments, care coordination and advocacy for clients transitioning from hospital and navigating complex services in the Melbourne region.

You will work with aged care facilities, health professionals and families to ensure person-centred, integrated care, supporting independence and wellbeing across the community.

Qualifications

• Qualified Social Worker with eligibility of the AASW.
• Demonstrated experience in completing comprehensive Social Work assessments.
• Experience working with at-risk and vulnerable people with complex needs in subacute/community health settings and aged care services.
• Advanced knowledge of Social Work theory (e.g. crisis intervention, strengths-based).
• Well-developed written, verbal, interpersonal and computer literacy and communication skills
• Demonstrated ability to manage multiple competing tasks and effectively prioritise.

Responsibilities

• Provide psychosocial assessment, care coordination, advocacy and case management to support clients transitioning from hospital.
• Collaborate with Residential Aged Care Facilities, health professionals, community providers, clients and families to deliver person-centred, integrated care.
