Job summary

Bayside Health in Frankston, Victoria is seeking a Community Care Social Worker to provide psychosocial assessment, care coordination, advocacy, and case management for clients transitioning from hospital settings.

You will work with residential aged care facilities, health professionals, clients and families to deliver person-centred, integrated care aimed at maintaining independence and wellbeing.

Qualifications

• Qualified Social Worker with eligibility of the AASW.
• Experience completing comprehensive Social Work assessments.
• Experience with at-risk and vulnerable people with complex needs in subacute/community health and aged care.
• Advanced knowledge of Social Work theory (e.g., crisis intervention, strengths-based).
• Strong written, verbal, interpersonal and computer literacy.

• Fixed Term Part Time, 40 hours per fortnight until June 2027
• Located in Frankston, Victoria
• Classification: Social Worker Grade 2, Year 1 - Year 4 (SC21 - SC24)* plus super

The Team & Role

Be part of a dedicated team committed to supporting older people and those with complex health needs to live safely and well. As a Community Care Social Worker, you will provide psychosocial assessment, care coordination, advocacy, and case management to support clients transitioning from hospital, helping them navigate services, access the right supports, and maintain their independence and wellbeing. Working closely with Residential Aged Care Facilities, health professionals, community providers, clients, and families, you will make a meaningful difference through person-centred, integrated care.

Skills & Experience Required

• Qualified Social Worker with eligibility of the AASW.
• Demonstrated experience in completing comprehensive Social Work assessments
• Demonstrated experience working with at-risk and vulnerable people with complex needs in subacute/community health settings and aged care services.
• Advanced demonstrated knowledge and application of Social Work theory (e.g. crisis intervention, strengths based).
• Well-developed written, verbal, interpersonal and computer literacy and communication skills
• Demonstrated ability to manage multiple competing tasks and effectively prioritise.
