Job summary

Bayside Health in Frankston, Victoria is seeking a Community Care Social Worker to provide psychosocial assessment, care coordination, advocacy, and case management for clients transitioning from hospital settings.

You will work with residential aged care facilities, health professionals, clients and families to deliver person-centred, integrated care aimed at maintaining independence and wellbeing.

Qualifications

• Qualified Social Worker with eligibility of the AASW.
• Experience completing comprehensive Social Work assessments.
• Experience with at-risk and vulnerable people with complex needs in subacute/community health and aged care.
• Advanced knowledge of Social Work theory (e.g., crisis intervention, strengths-based).
• Strong written, verbal, interpersonal and computer literacy.

Responsibilities

• Provide psychosocial assessment and care coordination for clients.
• Offer advocacy and case management to support hospital discharge and transitions.
• Collaborate with residential aged care facilities and health professionals.
• Support clients to navigate services and maintain independence and wellbeing.
