Job summary

UnitingCare Queensland, BlueCare Community Care in the Toowoomba region, seeks a Clinical Care Partner to oversee and coordinate complex clinical needs, delivering person-centred care to clients in home and community settings. You will develop and review care plans with clients and multidisciplinary teams, manage budgets and resources, and ensure timely responses to changing care needs.

Qualifications

• Registered Nurse or Allied Health professional with a current AHPRA registration.
• Understanding of client-centered, community-based care delivery.
• Experience with community funding streams and programs.
• Excellent communication and clinical problem-solving skills.
• Current Australian Driver’s Licence and ability to drive a work vehicle.
• Demonstrated ability to work autonomously and manage time.

Responsibilities

• Develop and review complex care plans in collaboration with the client and multidisciplinary care team
• Manage budgets and facilitate multidisciplinary care activities
• Liaise with clinical teams to monitor and respond to changing needs of complex community clients
• Continuously monitor, review and evaluate care plans, services and outcomes
• Assist clients in navigating the care system and linking them to resources where required
