The role involves driving outcomes through collaboration and on-road client engagement. Key requirements include a Certificate IV in Health or Community Service, a current Australian driver’s licence, and the ability to work autonomously in diverse environments.

Qualifications

• Certificate IV in a Health or Community Service discipline is essential.
• Demonstrated understanding of client centred, community-based care services delivery.
• Excellent communication and clinical problem-solving skills.

Responsibilities

• Develop and review care plans with clients and care team.
• Manage budgets and coordinate multidisciplinary care activities.
