Job summary

UnitingCare Children, Young People and Families – Bowen QLD is seeking a Care Partner to deliver person-centred, high-quality community care across Bowen, Proserpine and surrounding areas. You will oversee care planning, coordinate services, and work with clinical teams to achieve the best outcomes for each client, often on the road visiting clients in the community.

The role requires a Certificate IV in Health or Community Services, a current Australian driver's licence, a roadworthy vehicle

Qualifications

• Certificate IV in Health or Community Service (Essential).
• Current Australian Driver’s Licence (Essential).
• Roadworthy & insured vehicle (Essential).
• Experience with client-centred, community-based care delivery.
• Understanding of community funding streams.

Responsibilities

• Develop and review care plans with client and care team.
• Manage budgets and coordinate multidisciplinary activities.
• Liaise with clinical teams to monitor changing needs.
• Monitor and evaluate care plans and outcomes.
• Assist clients in navigating the care system and resources.
• Spend majority of day on the road engaging with clients in the community.
