Job summary

BlueCare in Caboolture is seeking a Clinical Care Partner to coordinate complex community care. You will assess needs, plan care, and work with multidisciplinary teams to deliver high-quality, person-centred services.

You will travel to client sites, manage care plans, and ensure outcomes align with funding. RN or Allied Health with AHPRA registration is essential, along with a valid driver’s license and autonomous work style.

Qualifications

• Registered Nurse or Allied Health professional with AHPRA registration.
• Strong communication and clinical problem-solving skills.
• Valid Australian driver's license and a roadworthy vehicle for work tasks.

Responsibilities

• Oversee and coordinate complex clinical care needs for clients in the community.
• Develop and review complex care plans with clients and multidisciplinary teams.
• Manage budgets and coordinate multidisciplinary care activities.
• Liaise with clinical teams to monitor changing needs and review outcomes.
• Assist clients in navigating the care system and link to resources.
