Job summary

Northeast Health Wangaratta (NHW) in Wangaratta, VIC, seeks a Community Care Intake Advisor to support timely responses to referrals for NHW community clinics and services. The role forms the central intake point, applying eligibility guidelines and triage as delegated, while coordinating with medical, nursing and allied health staff.

Qualifications

Responsibilities

• Support timely response to and management of referrals to NHW community clinics and services.
• Act as integral part of the Community Care Intake team for service access and triage.
• Apply eligibility and intake guidelines to referrals as part of the intake process.
