Job summary

Northeast Health Wangaratta (NHW) is the major health provider in the Central Hume region, delivering acute, sub-acute, aged care and community services. The Community Care Intake Advisor (CCIA) role supports timely responses to referrals to NHW community clinics and services, serving as a central point for service access and applying intake guidelines to referrals.

The CCIA works closely with medical, nursing and allied health clinicians and helps implement a priority framework to ensure timely
