Northeast Health Wangaratta (NHW) is recruiting a Community Care Intake Advisor to support timely responses to referrals for NHW community clinics and services. The role is part-time, 6 months fixed term, 48 hours per fortnight, and requires strong organisation and communication with clinical teams.

You will be part of the Community Care Intake team, applying eligibility guidelines and assisting with clinical prioritisation to ensure timely access to care for the region.

Qualifications

• High level of organisation and attention to detail.
• Exceptional communication experience with medical, nursing and allied health clinicians.
• Experience managing referrals and eligibility intake guidelines.

Responsibilities

• Support timely response to referrals to NHW community clinics and services.
• Apply eligibility and intake guidelines for referrals.
• Contribute to a clinical prioritisation framework to support timely triage.
