Neami National is seeking a Community Care Coordinator - Lived Experience in NSW. The role focuses on one-on-one psychosocial support, care coordination, and connection to peer and clinical services, drawing on lived experience to foster recovery and hope.

Community Care Coordinator - Lived Experience

Lived Experience Community Care Coordinator

For all people in Australia living with mental health and wellbeing challenges to live a life that is meaningful to them.

• Full-time (5 days a week), Permanent Position
• Classification: SCHADS Level 4 with a salary range of $92,378 - $99,393 per annum + Superannuation

About your new role
Funded by the Central and Eastern Sydney Primary Health Network, The Way Back -Universal Aftercare- is a non-clinical, psychosocial support service that provides compassionate and non-judgemental follow-up care to people and their loved ones after a suicidal crisis or suicide attempt

Community Care Coordinators-Lived Experience draw on their lived or living experience(s) of suicide and/or experiences of distress, to instil confidence and hope in others. Community Care Coordinators-Lived Experience will provide practical, emotional, social, and service access support that explores and responds to a person's needs through a process of mutual listening, learning, doing, being, belonging and healing.

Drawing on the National Lived Experience (Peer) Workforce Guidelines, the Lived Experience lens is:

• Learnings from personal experience of mental health challenges, distress and periods of healing, which may include service use
• Collective knowledge of universal lived experiences such as stigma, marginalisation, loss of power, and system hurt, as well as hope, healing, and recognising personal recovery as real and individual
• Professional principles and values such as mutuality, intersectionality, autonomy, hope and sharing power
• And the history of the Consumer movement, advocacy, and actions.

Community Care Coordinators-Lived Experience will draw on their Lived Experience learnings to provide short-term, one-on-one, flexible, psychosocial support and care coordination to:

• Co-design personal safety plans with participants and allies, which may include multi-agency care
• Provide one-on-one service navigation, coordination and guidance to participants and carers regarding wellbeing, wider services, and other available Peer spaces by drawing on the Lived Experience movement and peak bodies, alongside wider resources and local/community opportunities which may include clinical support
• Support participants with building a strong therapeutic alliance based on their needs, ensuring continuity of care, including engagement with the individuals GP, or in the case where there is not a regular GP, supporting them to link with one
• Support participants to explore their goals and engage with other relevant clinical, community and local services and supports.
• Engage the persons support network, providing resources and culturally appropriate support where required.
• Provide follow up to participants after they leave the service as required to ensure people remain linked with their appropriate support network to facilitate ongoing care and wellbeing.

Community Care Coordinators- Lived Experienceat Neami receive Intentional Peer Support (IPS) Training, Lived Experience Discipline Specific Supervision, access to a community of peer workers through Neami's Lived Experience Communities of Practice and are provided with three days of Mental Health and Wellbeing Leave. Community Care Coordinators-Lived Experience will be welcomed into an organisation that highly values peer work and in which all managers undertake IPS for Manager's training to support LE workforce development and integration into all levels of service delivery.

What you will bring to the role

• Relevant tertiary qualification and/or equivalent experience and understanding of working with people experiencing distress, with mental health challenges, suicidality and/or deliberate self-harm.
• Ability to establish and maintain positive working relationships with partner agencies and organisations to strengthen community-based prevention and intervention initiatives.
• Strong emotional resilience including the ability to sit with discomfort, distress andpeople in crisis.
• Experience liaising with external service providers, including specialist mental health services, GP's and other community-based services and organisations.
• Personal lived/living experience of recovery as a family, friend or carer. Your recovery experience may be related to mental health, suicidal crisis, drugs and alcohol use or trauma
• Willingness to identify or be identified by others as a professional with lived experience of recovery
• Willingness to share and draw on lived experience purposefully in the role to instill confidence and hope in others about the journey of recovery

• Nationally Coordinated Criminal History Check (police check) - disclosable outcomes considered
• Current Working with Children Check - required before commencement of work
