SA Health's Limestone Coast Local Health Network in Mt Gambier is seeking a Community Care Coordinator to support community-based aged care, disability and home support services. You will coordinate, implement and evaluate services to help consumers stay independent at home.

Working in a multidisciplinary team, you will supervise staff, ensure quality and compliance, and build effective relationships with health professionals, government agencies and community providers.

Qualifications

• Demonstrated managerial or supervisory experience.
• Experience in aged care, disability or community health services.
• Strong organisational, problem-solving and decision-making skills.
