Job summary

NSLHD's Royal North Shore Hospital is seeking a part-time Social Worker to join the Community Aged Care team, delivering person-centred care to older adults in the community.

The role focuses on assessment, counselling, crisis management and care planning, with opportunities for professional development within a supportive team. This temporary position runs until 6 February 2028, with potential ongoing employment.

Qualifications

• Social Work degree and eligibility for the Australian Association of Social Workers (AASW).
• Demonstrated understanding of ageing, age-related illnesses, rehabilitation, accommodation placements, guardianship and end of life care.
• Experience in assessment, counselling, crisis management, case management, advocacy and referrals.

Responsibilities

• Provide a high quality Social Work service to consumers, carers and families to ensure optimal health outcomes.
• Contribute to person-centred care and build strong community connections.
• Promote culturally safe practice for Aboriginal and/or Torres Strait Islander peoples and other communities.
