Job summary

EnableU Health Solutions in Sydney, NSW is hiring a Registered Nurse for a permanent full-time role delivering community-based care across NDIS, Support at Home, and DVA programs. You will assess, plan, implement and evaluate nursing care during home visits.

Our team of nurses, allied health professionals and care staff supports clients to stay well, independent and connected to their community in their own homes. We work across NDIS, Support at Home, community nursing and DVA programs, and we take pride in care that is clinically sound, practical and centred on each person's goals. We're looking for a Registered Nurse who wants to make a real difference in community care.

Key Responsibilities include: Assessing, planning, implementing and evaluating nursing care for clients living in the community across NDIS, Support at Home, community nursing and DVA programs, in line with accepted nursing practice and professional standards. Conducting comprehensive clinical assessments during home visits, including falls risk, skin integrity, wound, continence, nutrition and cognition, and developing and reviewing individualised care and nursing plans.

Providing clinical interventions, treatments and therapies, including administering medications, wound management and clinical observations, and monitoring client responses to treatment and care plans. Identifying clinical deterioration and escalating concerns to GPs, allied health professionals and families, and responding to clinical incidents and risks. Working with physiotherapists, occupational therapists, GPs, support coordinators and other members of the multidisciplinary team to coordinate care.

Managing and coordinating a community caseload so care is responsive, goal-oriented and aligned with each client's plan and funding. Supporting NDIS participants with complex health needs through nursing assessments, health management plans and input into plan reviews. Promoting health and preventing illness through education for clients, families and carers on chronic condition management, medication safety, falls and pressure injury prevention, and early warning signs.

Giving clients and families clear, accurate information about their treatment, medications, care plans and ongoing health management. Supervising, supporting and delegating to Enrolled Nurses and support workers, including clinical guidance and competency checks. Contributing to staff training and supporting clinical governance through internal audits, incident review and continuous quality improvement.

Requirements: Bachelor degree in Nursing. Current registration as a Registered Nurse with AHPRA.

Experience in community nursing, home care, aged care or NDIS settings desired. Sound knowledge of nursing principles and their practical application. Strong interpersonal and communication skills.

Ability to travel to client homes across Sydney. Valid driver's licence and access to a reliable vehicle. National Police Check and NDIS Worker Screening Check.
