Job summary

Trilogy Care is seeking a senior clinical nurse to provide high-level clinical leadership and supervision for Team Leaders and Care Partners in Brisbane. You will act as an escalation point for complex cases, support incident management, and mentor staff to strengthen clinical capability.

In this role you will ensure care planning aligns with the Aged Care Act and Quality Standards, partnering with Clinical Governance to drive continuous improvement and promote a culture of safety and clinical

Qualifications

• Current registration as a Registered Nurse with AHPRA.
• Experience in general medical nursing with strong assessment and care coordination skills.
• Experience managing clients with chronic/complex conditions in community, hospital or aged care settings.
• Knowledge of gerontology, frailty, dementia and cognitive impairment.
• Aged care, community nursing or chronic disease management experience is highly regarded.

What you’ll do

• Strengthen clinical capability through mentoring and targeted training
• Ensure care planning aligns with the Aged Care Act and Quality Standards
• Partner with Clinical Governance to drive continuous improvement
• Contribute to a culture of safety, accountability and clinical excellence
• This role has no direct reports but holds significant clinical influence and leadership responsibility.

What you’ll bring

• Current registration as a Registered Nurse with Australian Health Practitioner Regulation Agency.
• Demonstrated experience in general medical nursing with strong assessment, clinical decision-making and care coordination skills.
• Experience managing clients with chronic and complex health conditions across community, hospital or aged care settings.
• Sound knowledge of gerontology, healthy ageing, frailty, dementia, cognitive impairment and the clinical issues affecting older people.
• Experience in aged care, community nursing, care coordination or chronic disease management is highly regarded.
• General medical nursing experience with the ability to assess, monitor and manage clients with chronic and complex health conditions.
• Demonstrated experience in the care of older people, including chronic disease management, gerontology, frailty, dementia and cognitive impairment.
• Experience undertaking comprehensive clinical assessments, recognising deterioration, coordinating multidisciplinary care, and supporting evidence-based clinical decision-making.
• Highly developed communication skills with the ability to provide clinical advice and support to clients, carers, general practitioners and other healthcare providers in a remote care environment.
