Job description

Everyone has a different idea of what living well looks like. For some, it's staying active. For others, it's keeping their routines, enjoying their independence or simply feeling confident in their own home.

At Trilogy Care, we help make those things possible.

As one of Australia's fastest growing approved Support at Home providers, we're bringing fresh thinking, smart technology and a more personal approach to aged care.

We're looking for Assessment Partners who are curious, compassionate and great at connecting with people. You'll get to know our clients, understand what matters to them and turn those conversations into personalised care plans that make a real difference.

You don't need to be a clinician to thrive here. If you're a confident communicator with strong problem solving skills and a genuine interest in helping people, this could be your opportunity to build a meaningful career in aged care.

The Role

You'll be the person who helps turn a client's needs and goals into a practical plan for living well at home. Through phone and video assessments, you'll ask the right questions, identify risks, recommend suitable services and develop care plans and budgets that give clients more choice and control. It's a role that combines meaningful conversations, critical thinking and practical problem solving, with the opportunity to make a difference every day.

What You'll Do
  • Conduct personalised client assessments, understanding individual needs, goals and preferences
  • Develop tailored care plans and budgets aligned with Support at Home funding
  • Identify potential risks, recommend appropriate supports and arrange referrals where needed
  • Guide clients and families through care options, helping them make informed decisions
  • Manage a busy schedule of assessments while maintaining accurate records and documentation
  • Collaborate with internal teams to deliver safe, consistent and high quality care
What You'll Bring
  • Strong communication skills and a natural ability to connect with people
  • Experience in customer service, aged care, NDIS, healthcare or a similar environment
  • Confidence asking questions, solving problems and navigating sensitive conversations
  • Great organisational skills and the ability to manage competing priorities
  • Confidence using technology and learning new systems
  • A proactive attitude, sound judgement and a genuine interest in helping others
Why Trilogy Care?
  • Monday to Friday only - no shiftwork, no weekends
  • No driving required - all client interactions are via phone or video
  • Modern office in King Street, Bowen Hills - surrounded by cafes, supermarkets, and public transport
  • Clear career pathways across departments for those who show initiative
  • Salary packaging options to boost your take-home pay
  • Vibrant, inclusive team culture with regular social events
  • Employee benefits including Fitness Passport, EAP support for you and your family
Help Us Shape the Future of Aged Care

Let this be the role that brings purpose and balance back into your career. Empower older Australians to live independently in the home they love for longer and help us raise the standard of care, one assessment at a time.

At Trilogy Care, we're changing aged care for good, not with more paperwork, but with smarter, faster, AI-supported ways of working that put clients and employees first. With organisation-wide access to AI tools and strong leadership support, every team member is empowered to embrace new ways of working. By lifting the burden of admin, AI is helping us make workdays more human, not less. If you've ever wished for more time with clients and less time on documentation, you'll find a future-focused care team here that's ready to back you all the way.

Please Note

Applicants must have full Australian working rights. A police check and professional references are required as part of our pre employment process.

We appreciate the interest of recruitment agencies, but kindly ask that resumes not be submitted unless a prior agreement is in place. Trilogy Care will not accept responsibility for any fees related to unsolicited resumes.
