HammondCare, a not-for-profit charity delivering aged, home, dementia and palliative care, seeks a part-time Home Care Registered Nurse (Clinical Partner) in the Hunter–Cardiff region. You will assess needs, develop and review care plans, and deliver clinical care in clients’ homes, traveling between HammondCare sites as required.
Applicants must be an AHPRA-registered RN, with strong communication, teamwork and advocacy skills.
HammondCare Home Care is currently looking for an experienced permanent, part time, Home Care Registered Nurse – Known within HammondCare as a Clinical Partner to support the team in the Hunter - Cardiff region 3 days a week.
The Clinical Partner (Home Care Registered Nurse) works within HammondCare’s Mission in Action to deliver high-quality, person-centred clinical care that supports clients to live well in their own home. The role provides evidence-based, best practice clinical care holistically, to individuals, ensuring their unique goals, needs and preferences are understood and addressed. This role is focused on assessing clinical care needs, developing, and reviewing care plans and delivering excellent clinical care in partnership with clients and key stakeholders. The role works collaboratively with the care management team to ensure the best outcomes for clients in the community setting and provides guidance to care staff to ensure the safe and continuous delivery of high-quality clinical care. The clinical partner will be travelling to Cardiff , Waratah, and surrounding suburbs in the Newcastle region.
