This is a Permanent, full-time role based in the South Coast Region, with an office located at Merimbula.
As Clinical Partner – Registered Nurse, you’ll be a valued member of our growing Home Care Clinical Care & Practice Team, working closely with an experienced, multidisciplinary team to provide tailored and individualised services to older adults living at home.
A typical day will involve assessing functional care needs, setting care goals, and providing treatment and intervention, helping people stay active, independent and right where they belong – home.
You’ll be provided with a broad and varied caseload, with support from experienced clinicians.
This meaningful role will include performing home visits or running group activities for clients accessing Support at Home or CHSP services or residing in one of our Seniors Living units.
You will deliver person-centred care in an environment that promotes continuous quality improvement, consistent with the values of HammondCare and the Australian Health Practitioners Regulatory Agency (AHPRA).
