Job summary

HammondCare is seeking a permanent part-time Clinical Partner - Nursing to support clients living at home in the Central West/Nepean/Blue Mountains region. This role is three days per week, Monday to Friday, within the Home Care Clinical Care & Practice Team to assess needs, set goals and deliver tailored, person-centred care in clients’ homes.

You will work with an experienced multidisciplinary team to provide care, perform home visits or group activities, and contribute to continuous quality

Qualifications

• Registered Nurse with current AHPRA registration.
• Experience in aged care, inpatient, community or home-based settings is desirable.
• Strong communication and time-management skills.

Responsibilities

• Assess client care needs and set tailored care goals.
• Deliver person-centred nursing care in home settings.
• Coordinate with multidisciplinary team and document outcomes.
• Travel between client homes to provide home-based care.

This is 3 days per week between Monday - Friday.As **Clinical Partner – Registered Nurse,** you’ll be a valued member of our growing Home Care Clinical Care & Practice Team, working closely with an experienced, multidisciplinary team to provide tailored and individualised services to older adults living at home.A typical day will involve assessing functional care needs, setting care goals, and providing treatment and intervention, helping people stay active, independent and right where they belong – home. You’ll be provided with a broad and varied caseload, with support from experienced clinicians. This meaningful role will include performing home visits or running group activities for clients accessing Support at Home or CHSP services or residing in one of our Seniors Living units.
