Job summary

St Vincent's Health Australia is seeking a Social Worker (Level 1-2) for St Vincent’s Public Hospital, Sydney. This permanent, full-time position offers a salary of $79,419 to $113,301 plus super and salary packaging.

Join a dedicated team in the Heart Lung Stream, providing psychosocial assessment, family support, and discharge planning. You will collaborate with multidisciplinary colleagues and may participate in after-hours on-call duties as required.

Qualifications

  • Hold a degree in Social Work and be eligible for membership with the Australian Association of Social Workers (AASW).
  • Strong clinical skills in psychosocial assessment, counselling, and case management.
  • Experience in a tertiary hospital or health-related setting preferred.
  • Ability to work within multidisciplinary teams in high-pressure environments.

Responsibilities

  • Provide social work services to inpatients, families and carers across cardiology and related specialities.
  • Participate in multidisciplinary meetings to support patient care and referrals.
  • Advocate for patients and carers, including guardianship and well-being applications as needed.
  • Contribute to documentation, handovers and on-call duties.

About The Role
  • Working in the Heart Lung Stream, providing social work services to inpatients, families, and carers within Cardiology , Cardio-Thoracic Surgery, Thoracic Medicine, Thoracic and Cardiac Rehabilitation and Heart and Lung Transplant including psychosocial assessment, risk assessment, counselling, and family support
  • Participate in multidisciplinary team meetings to support patient care, discharge planning, and coordination of referrals to community supports, accommodation, and placement services
  • Advocate for patients and carers, including support with NCAT Guardianship and NDIS applications where required
  • Contribute to clinical operations including documentation, handovers, statistical reporting, clinical cover, on-call duties, and resource development
  • Engage in professional development activities including supervision, education, research, student training, quality improvement, and mandatory training compliance
About You
  • Hold a degree in Social Work and be eligible for membership with the Australian Association of Social Workers (AASW), with demonstrated clinical experience in a tertiary hospital or health-related setting
  • Strong clinical skills and knowledge in psychosocial assessment, counselling, therapeutic interventions, case management, advocacy, and complex discharge planning
  • Understanding of and practical experience in crisis intervention, short-term solution-focused approaches, and grief and loss support
  • Ability to work effectively within multidisciplinary teams in high-pressure environments, managing competing demands with strong negotiation and liaison capabilities
  • Willingness and ability to participate in the after-hours on-call hospital social work crisis service as required
