Job summary

Future Focus Group is seeking a Clinical Lead to guide the Melbourne East Team from Ringwood, delivering Specialist Support Coordination with a person-centred approach.

You will mentor Support Coordinators, contribute to practice development, and strengthen clinical services through evidence-based approaches and collaboration across Health, Justice, Mental Health, and Housing sectors.

Qualifications

• Bachelor’s level qualification in Social Work, Social Science, Health Science, Case Management, Mental Health/Psychology or other Allied Health qualification.
• Experience supporting participants in the NDIS with mental health, psychosocial or disability needs in a case-management style role.
• Extensive knowledge of disability, mainstream and community services.

Responsibilities

• Provide Clinical Supervision and risk reviews across the assigned region.
• Support ongoing professional development for Support Coordinators in care.
• Manage a small caseload of participants with complex needs and support implementation of NDIS plans.
