Job summary

Mission Australia in Lismore, NSW is seeking a Clinical Coordinator to oversee complex client care and drive recovery‑oriented practice within the PSP across Tweed Heads, Lismore, Grafton and Coffs Harbour.

The on‑site role partners with the Program Manager and health and community services, delivering tailored assessments, case planning and risk reviews to support people living with mental illness.

Qualifications

• Bachelor degree in Social Work, Psychology, Occupational Therapy, Mental Health Nursing or a related discipline.

Responsibilities

• Provide clinical supervision to frontline staff to strengthen service quality and outcomes.
• Oversee complex client matters, offering clinical advice and ensuring safe, recovery-focused service delivery.
• Conduct mental health and intake assessments, clinical reviews and risk assessments; support care plan development.
• Lead risk escalation processes for complex clients with timely follow-up and interventions.
• Coordinate collaborative care planning and case conferences with participants, families, clinical teams and external providers.
• Build partnerships with community organisations and clinical services to enhance integrated delivery.

Your Opportunity

The Psychosocial Support Program (PSP) is a vital program in the community in partnership with the Healthy North Coast PHN supporting people living with mental illness.

This opportunity is to cover until 03/05/2027, with the possibility of extension.

As the Clinical Coordinator, you will be the driving force behind our commitment to providing recovery-oriented support, ensuring every individual receives tailored care through a comprehensive stepped care approach.

Collaboration and cooperation are the heart of our operation, and you will be working closely with the Program Manager, Team Leader and a broad network of health and community services. This will see you overseeing complex client care, supporting clinical decision making, facilitating assessments and risk reviews, and strengthening collaborative service delivery across Tweed Heads, Lismore, Grafton and Coffs Harbour. This is an on-site position with flexibility to be based from either Tweed Heads, Lismore, Grafton or Coffs Harbour. Together, you will deliver an integrated client journey from referral through to intake and assessment, ensuring timely access to appropriate support and services

Moreover, your expertise will extend beyond administrative excellence. This is a role that is instrumental in the development of case plans and recovery goals, working hand in hand with our dedicated Recovery Coordinators Access Enablers, Youth Wellbeing Coordinators, and Peer Advocates. Through your guidance, you will empower individuals on their unique recovery journeys, fostering resilience and hope every step of the way on the path to wellness.

Your Key Responsibilities

• Provide clinical supervision, guidance and support to frontline staff to strengthen service quality, capability and participant outcomes.
• Oversee complex client matters, offering clinical advice and ensuring safe, effective and recovery focused service delivery.
• Conduct mental health assessments, intake assessments, clinical reviews and risk assessments, supporting the development and review of care plans and recovery goals.
• Lead risk escalation processes for complex clients, ensuring timely follow up, risk management and appropriate support interventions.
• Coordinate collaborative care planning and case conferences with participants, families, clinical teams and external service providers.
• Build and maintain strong partnerships with community organisations, clinical services and key stakeholders to enhance integrated service delivery.
• Contribute to continuous quality improvement activities, service development initiatives and the review of clinical practices, policies and procedures.

Requirements for Success

• A bachelor’s degree in Social Work, Psychology, Occupational Therapy, Mental Health Nursing or a related discipline.
• A minimum of two years’ experience supporting vulnerable people experiencing complex and severe mental health challenges.
• Demonstrated experience conducting mental health assessments, risk assessments, care planning and risk management processes.
• NDIS experience would be desirable.
• Strong understanding of recovery oriented practice, trauma informed care, person centred support and the social determinants of health.
• Ability to work creatively and safely with clients from diverse backgrounds, including LGBTIQ, CALD and Aboriginal and Torres Strait Islanders.
• Proven ability to work collaboratively within multidisciplinary teams while building effective relationships with participants, stakeholders and service providers.
• Well developed communication, problem solving and clinical decision making skills, with the ability to manage complex and sensitive situations.
• Comfortable with ambiguity and juggling competing priorities, and able to work independently and as part of a multi-disciplinary team.
• A commitment to self reflection, continuous learning and ongoing improvement in clinical practice and service delivery.
• Full driver licence with no restrictions
