Job summary

Mission Australia is seeking a compassionate Case Worker to join the Happy Healthy Minds Program in Cooma NSW. You will deliver early intervention support to children, young people and families aged 0-18, applying trauma-informed, family-centred approaches.

You will provide school-based, home-based and community outreach support, developing individual case plans, coordinating referrals, and delivering psychoeducational programs while maintaining accurate documentation and partnering with service

Qualifications

  • Experience using trauma-informed, family-centred and strengths-based approaches.
  • Ability to develop individualized case plans and coordinate services.
  • Strong written and oral communication with robust documentation.
  • Willingness to undertake police check and working with children checks.

Responsibilities

  • Deliver trauma-informed, client-centred case management to children, young people and families.
  • Respond to referrals and conduct needs assessments with home visits/outreach.
  • Develop and implement individualized case plans and facilitate referrals.
  • Coordinate and deliver therapeutic and psychoeducational programs.
  • Maintain accurate case notes, risk assessments and reports.
  • Collaborate with internal teams and external agencies for client advocacy.

  • $81,903 to $86,066 per annum + Super + up to $15,900 NFP Salary Packaging benefit

Your Opportunity

Mission Australia is seeking a compassionate, skilled and motivated Case Worker to join our Happy Healthy Minds Program, supporting children, young people and families across the Cooma region. This rewarding role is ideal for professionals who are passionate about improving mental health outcomes for vulnerable children and young people and have experience working within trauma-informed, family-centred and strengths-based frameworks.

As a Case Worker, you will deliver early intervention support to children and young people aged 0-18 years who are experiencing, or at risk of experiencing, mental health challenges. You will work closely with families, schools, community organisations and other service providers to provide practical support, therapeutic interventions and coordinated care that helps clients build resilience and thrive.

You will provide a combination of school-based, home-based and community outreach support through both short-term and longer-term case management services.

Your key responsibilities will be to:
  • Deliver trauma-informed, client-centred case management to children, young people (0-18), and families experiencing or at risk of mental health issues.
  • Respond to referrals, conduct initial needs assessments, and provide ongoing support through home visits and outreach services.
  • Develop and implement individualized case plans, provide practical support, and facilitate referrals to internal and external services.
  • Coordinate and deliver therapeutic and psychoeducational programs
  • Maintain accurate and timely case documentation, including risk assessments, progress reports, and data entry.
  • Work collaboratively with internal teams and external agencies to advocate for clients, participate in case reviews, and support community development.
Requirements for Success:
  • Tertiary qualifications and/or relevant experience in the human services industry, with an understanding of mental health.
  • Provide tailored case management through comprehensive assessments, developing individualized case plans, and facilitating ongoing support for clients.
  • Respond to client referrals promptly, ensuring timely contact, support, and effective service delivery to children, young people, and families.
  • Coordinate and deliver individual and group support programs, such as mental health education, and community outreach.
  • Excellent oral and written communication skills, with the ability to maintain accurate case documentation and engage effectively with clients and external agencies.
  • Current NSW driver's license
  • Current First Aid and CPR Certificate

Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
