Working closely with multidisciplinary teams including nurses, allied health clinicians and service staff, you will coordinate clinical care, provide case management support and ensure the right interventions are delivered at the right time. • Demonstrated experience in clinical aged care, rehabilitation, restorative care or complex case management.

  • Strong clinical judgement and the ability to identify and elevate risks or changing care needs.

  • Excellent communication and collaboration skills when working with multidisciplinary teams.

  • Ability to work autonomously in a fast‑paced environment with changing priorities.

We would also love you to have:

  • Experience working within Transitional Care Programs (TCP) or hospital‑to‑home rehabilitation services.

  • Knowledge of aged care legislation, standards and community aged care programs.

  • Experience supporting customers with complex conditions including wound care, medication coordination or palliative care.
