Job summary

Australian Unity Limited is seeking a Clinical Care Partner to support safe, coordinated care for customers with complex needs. This permanent full-time role offers hybrid working with three days in the Oxley office and two days from home.

You will provide clinical leadership, escalation management, and decision support to Care Partners and the multidisciplinary team, helping identify risk and improve customer outcomes through integrated care planning.

Qualifications

• 4+ years' experience supporting older people in community, subacute or complex care settings.
• Tertiary qualifications in Nursing or Allied Health with current AHPRA registration where applicable.
• Provide clinical leadership, escalation management and decision support to Care Partners and multidisciplinary teams.
• Identify and manage clinical risk and deterioration to support customer outcomes.
• Support transitions of care including hospital admissions, discharges and changes in customer needs.

Responsibilities

• Provide clinical leadership and escalation support for customers with complex, unstable, or changing needs
• Support Care Partners to identify, assess, and respond to clinical risk and deterioration
• Undertake clinical reviews and interventions to optimise customer outcomes
• Apply clinical judgement to support safe and timely decision making
• Support transitions of care, including hospital admissions, discharges, and changes in customer needs
• Facilitate collaboration across Care Partnering, Service Delivery, Clinical Services, and external healthcare providers
• Assist with clinical governance, risk management, and quality improvement activities
• Contribute to workforce capability through coaching, mentoring, and sharing clinical expertise
• Support proactive care planning and clinical oversight for higher-risk customer cohorts
• Ensure accurate documentation and appropriate use of clinical systems and escalation pathways
