Job summary

Southern Cross Care (NSW & ACT) invites applications for a full-time Clinical Care Coordinator at our Cootamundra facility. You will support Registered Nurses and care staff in clinical management, maintain high standards of care, and promote a positive work environment.

Responsibilities include monitoring residents' care plans, advising on complex nursing needs, driving continuous improvement, updating documentation, and liaising with doctors, hospitals and other health professionals.

Qualifications

• Experience in clinical care coordination or nursing environment is desirable.
• Ability to monitor and adapt care plans based on resident needs.

Responsibilities

• Monitor the care plans of residents
• Act as an adviser in response to complex nursing and care needs
• Drive continuous improvement to maintain high standards of client care
• Monitor the implementation of care plans and update as appropriate
• Manage each resident’s condition and ensure documentation reflects ongoing care
• Liaise with Doctors, Paramedics, hospitals and health professionals
