Job summary

Southern Cross Care (NSW & ACT) is seeking a dedicated Clinical Care Coordinator for our Cootamundra facility. The role supports Registered Nurses and care staff in clinical management, quality care delivery, standards maintenance and a positive work environment.

You will monitor resident care plans, advise on complex nursing needs and drive continuous improvement to ensure high-quality outcomes. Strong leadership, English communication and numeracy are essential for success.

Qualifications

• Minimum 5 years’ Registered Nurse experience in aged care or health-related area.
• Strong leadership and ability to lead care teams.
• Excellent interpersonal and written English communication skills.
• Active role in delivering quality care to frail elderly residents.
• Numeracy skills for reconciliations and statistics.
• Sensitivity to and understanding of elderly needs in residential care settings.

Responsibilities

• Monitor the care plans of residents.
• Act as an adviser on complex nursing and care needs.
• Engage in continuous improvement to maintain high care standards.
• Review and update care plan implementations regularly.
• Manage residents’ conditions and progress with accurate documentation.
• Liaise with Doctors, Paramedics, hospitals and other health professionals.
