Job summary

Respect, a major aged care provider in Regional Australia, seeks a Clinical Care Coordinator for Riverside Gardens on the NSW Mid North Coast. You will mentor the care team, develop person-centred care plans, and deliver safe, high-quality clinical care in a collaborative environment.

With a Bachelor of Nursing and current AHPRA registration, you’ll have at least 2 years’ aged care experience, strong clinical assessment, documentation, and communication skills, and a commitment to accreditation

Qualifications

  • Bachelor of Nursing with current AHPRA registration.
  • Minimum 2 years’ nursing in residential aged care.
  • Ability to work collaboratively within multidisciplinary teams.
  • Strong clinical assessment and documentation skills.
  • Excellent communication and interpersonal abilities.
  • Proficient with Microsoft Office and clinical software.
  • Solid understanding of aged care accreditation requirements.

Responsibilities

  • Coordinate admission of new residents and develop person-centred care plans with healthcare professionals.
  • Lead and support the care team to deliver safe, high-quality care.
  • Provide hands-on nursing care including medication administration and wound care.
  • Maintain accurate clinical documentation and ensure infection control practices.
  • Communicate with residents, families, and external providers while driving quality improvement.

No two days are the same! As Clinical Care Coordinator you will:

  • Coordinate the admission of new residents, completing comprehensive assessments and developing person-centred care plans in collaboration with healthcare professionals.
  • Lead, mentor, and support the care team to deliver safe, high-quality clinical care and positive resident outcomes.
  • Provide hands-on nursing care, including medication administration, wound management, and ongoing clinical monitoring.
  • Maintain accurate clinical documentation, oversee infection control practices, and ensure compliance with regulatory standards and audits.
  • Communicate effectively with residents, families, and external healthcare providers while driving quality improvement initiatives.
About You
  • Bachelor of Nursing with current AHPRA registration.
  • Minimum 2 years’ nursing experience in residential aged care, with experience in a similar clinical care role highly regarded.
  • Demonstrated ability to work collaboratively within multidisciplinary care teams.
  • Strong clinical assessment, documentation, and evidence-based practice skills.
  • Excellent communication and interpersonal skills to engage effectively with residents, families, and colleagues.
  • Proficient computer and information management skills, including Microsoft applications and clinical software systems.
  • Sound understanding of the aged care accreditation process and ability to meet Aged Care Worker Screening legislative requirements.
