Rockhampton and Hervey Bay-based clinical care role with a respected community mental health provider seeks an experienced Clinical Care Coordinator. You will manage a clinical caseload, perform assessments, plan care and deliver evidence-based interventions in a multidisciplinary team.

Qualifications

• Minimum tertiary qualification in Psychology, Social Work, Mental Health Nursing, Occupational Therapy or related discipline.
• Current professional registration or eligibility relevant to your discipline.
• Current Australian driver's licence and ability to travel between sites.
• National Police Check and Working With Children Check where applicable.
• Full Australian working rights.

Responsibilities

• Conduct comprehensive assessments, case formulations and care planning.

You'll play a vital role in delivering person-centred care that promotes recovery, wellbeing and long-term positive outcomes.

The Role As a Clinical Care Coordinator, you will:

• Manage a clinical caseload and provide direct mental health support
