Job summary

Sharp & Carter are seeking a Clinical Care Coordinator in Queensland to join a multidisciplinary team delivering recovery-focused mental health care. You will conduct clinical assessments, care planning, counselling, case management and coordinate services to support recovery.

We look for experience with complex presentations, trauma-informed practice, strong communication, and current Australian working rights.

Qualifications

• Professional qualification in social work, mental health nursing or occupational therapy.
• Experience with complex mental health presentations.
• Trauma-informed, recovery-oriented practice.
• Excellent communication and stakeholder engagement.
• Current driver’s licence and ability for Working with Children Check.
• National Police Check and full Australian working rights.

Responsibilities

• Conduct comprehensive mental health assessments and risk assessments.
• Develop personalised treatment and recovery plans.
• Deliver evidence-based therapeutic and counselling interventions.
• Manage an ongoing clinical caseload.
• Coordinate care with families, GPs, hospitals, allied health providers, and community services.
• Support clients transitioning between community and tertiary services.
• Participate in case reviews, clinical supervision, and quality improvement activities.
• Advocate for client wellbeing through a person-centred and recovery-oriented approach.

As a Clinical Care Coordinator, you'll play a vital role in supporting people throughout their recovery journey by providing clinical assessments, care planning, counselling, case management, and coordination of services to ensure positive, sustainable outcomes.

About the Role

Working as part of a multidisciplinary team, you will:

• Conduct comprehensive mental health assessments and risk assessments
• Develop personalised treatment and recovery plans
• Deliver evidence-based therapeutic and counselling interventions
• Manage an ongoing clinical caseload
• Coordinate care with families, GPs, hospitals, allied health providers, and community services

What We're Looking For

We are seeking clinicians who have:

• Experience working with complex mental health presentations
• Strong counselling and therapeutic intervention skills
• A recovery-oriented and trauma-informed practice approach
• Exceptional communication and stakeholder engagement abilities
• Experience working collaboratively within multidisciplinary teams
• Understanding of culturally safe practices, particularly when working with First Nations communities
• Relevant tertiary qualification in:
• Social Work
• Mental Health Nursing
• Occupational Therapy
• Or a related discipline
• Current AHPRA registration (where applicable) or eligibility for AASW membership
• Current Driver's Licence
• National Police Check
• Working with Children Check (or ability to obtain)
• Full Australian working rights
