Healthii People Australia, in partnership with Open Minds, is seeking a Clinical Care Coordinator for adults in Rockhampton, QLD. This full-time role sits within a multidisciplinary team delivering comprehensive mental health care for adults with moderate to complex conditions.

The position focuses on high-quality care rather than KPI targets, with responsibilities including intake, risk assessments, safety planning, evidence-based interventions, and holistic case management.

Qualifications

• Qualifications in Social Work, Psychology, or MH Nursing.
• Accreditation with AASW or AHPRA.
• Knowledge of trauma-informed therapeutic frameworks.
• Experience supporting adults with moderate to complex mental health challenges.
• Team-oriented and collaborative approach.

Responsibilities

• Work within a multidisciplinary team.

Healthii People are proud to have partnered with Open Minds a leading NGO, who in collaboration with other Mental Health Services are leading the development and oversight of the Clinical Care Coordination Program across the Country to Coast (CCQ) Primary Health Network (PHN).

These services are designed to provide a comprehensive, integrated and holistic approach to mental health care for adults with moderate - complex mental health conditions.

As a Clinical Care Coordinator, you will work within a multi-disciplinary team that is not KPI centred but more focussed on providing high quality mental health care to the participants.
