Job summary

Richmond Fellowship Queensland (RFQ) is recruiting a Clinical Care Coordinator in Jimboomba, QLD. Based from the Jimboomba hub with spokes in Beenleigh, Beaudesert and Browns Plains, you will lead clinical intake, assess needs, and develop care plans alongside individuals, families and carers.

Qualifications

• A tertiary qualification in a mental health discipline such as nursing, psychology, or social work.
• Current AHPRA registration as a Psychologist or Mental Health Nurse, or eligibility for AASW membership.
• Experience in clinical assessment, care planning, and coordination for people with persistent and complex mental health needs.
• Strong understanding of recovery oriented, trauma informed, and culturally responsive practice.

• Coordinate care across services to ensure support is connected, responsive, and centred around individual needs.
• Monitor mental health, risk, and progress, responding early and adjusting care to support stability and recovery.

• Work collaboratively with multidisciplinary teams and external providers to enable integrated, holistic care.
• Maintain high quality documentation and contribute to clinical governance, supervision, and continuous improvement.

Wellbeing Connect is an integrated, stepped care mental health program supporting adults experiencing persistent, severe, and complex mental health challenges. Based from the Jimboomba hub, with spokes in Beenleigh, Beaudesert and Browns Plains, the service brings together clinical care coordination and psychosocial supports to provide person centred, recovery oriented and trauma informed care close to where people live.

The Clinical Care Coordinator is the first clinical connection into Wellbeing Connect, setting the tone for how care is experienced from the very beginning. This role works alongside individuals, families, and carers to understand what matters, build clear and meaningful care plans, and ensure support is timely, coordinated, and responsive as needs change.

Working at the centre of the care team, you will bring clinical insight, structure, and continuity to each person’s journey. It is a role that balances clinical judgement with human connection, supporting stability, responding to risk, and helping people move toward recovery with confidence and support around them.

Key Responsibilities

About You

You are a clinically capable and values driven professional who brings both expertise and empathy to your work. You are confident working with complexity and understand the importance of coordinated, person centred care.

You will bring:

• A tertiary qualification in a relevant mental health discipline such as nursing, psychology, or social work
• Current AHPRA registration as a Psychologist or Mental Health Nurse, or eligibility for AASW membership
• Experience in clinical assessment, care planning, and coordination for people with persistent and complex mental health needs
• Strong understanding of recovery oriented, trauma informed, and culturally responsive practice
• Experience working across services to support integrated care and continuity

You will demonstrate:

• Strong clinical judgement and the ability to respond to risk and changing needs
• The ability to build genuine, respectful relationships with individuals, families, and service partners
• Confidence working both autonomously and as part of a multidisciplinary team
