Sharp & Carter is seeking a Clinical Care Coordinator in Hervey Bay, QLD to join a supportive multidisciplinary team delivering recovery-focused mental health care. You'll conduct clinical assessments, develop personalised care plans, provide counselling, manage an ongoing caseload, and coordinate services with families, GPs, hospitals and community providers to achieve positive, sustainable outcomes.

Qualifications

• Holds a relevant tertiary qualification (Psychology, Social Work, Mental Health Nursing, Occupational Therapy or related discipline).
• Current AHPRA registration or eligibility for AASW membership where applicable.
• Current Driver's Licence, National Police Check, WWCC (or ability to obtain).
• Full Australian working rights.

• Conduct comprehensive mental health assessments and risk assessments.

As a Clinical Care Coordinator, you'll play a vital role in supporting people throughout their recovery journey by providing clinical assessments, care planning, counselling, case management, and coordination of services to ensure positive, sustainable outcomes.

About the Role

Working as part of a multidisciplinary team, you will:

• Conduct comprehensive mental health assessments and risk assessments
• Develop personalised treatment and recovery plans
• Deliver evidence-based therapeutic and counselling interventions
• Manage an ongoing clinical caseload
• Coordinate care with families, GPs, hospitals, allied health providers, and community services
• Support clients transitioning between community and tertiary mental health services
• Participate in case reviews, clinical supervision, and quality improvement activities
• Advocate for client wellbeing through a person-centred and recovery-oriented approach

What We're Looking For

We are seeking clinicians who have:

• Experience working with complex mental health presentations
• Strong counselling and therapeutic intervention skills
• A recovery-oriented and trauma-informed practice approach
• Exceptional communication and stakeholder engagement abilities
• Experience working collaboratively within multidisciplinary teams
• Understanding of culturally safe practices, particularly when working with First Nations communities

Essential Requirements
