This position emphasises comprehensive medication reviews, polypharmacy management and transitions of care within a dedicated aged care pharmacy team.

Qualifications

• Clinical pharmacist with aged care experience.
• Availability to work Sundays and Mondays.

Job description

As a clinical pharmacist, you will be a key part of our dedicated aged care pharmacy team, focused entirely on enhancing the health outcomes and quality of life for aged care residents. Moving away from traditional retail-facing operations, your role will center entirely on specialised clinical care within the residential aged care environment.

You will serve as an accessible, trusted clinical resource on-site, directly assisting nursing staff, medical practitioners, and families with complex medication inquiries, polypharmacy management, and transitions of care. Key responsibilities
