Lumia Care is seeking an experienced Care Coordinator to join our Adelaide team. You will coordinate Support at Home services, leverage CHSP/CHIS funding knowledge and work with Care Partners and frontline teams to deliver compliant, client-centered care.

Strong communication, budget awareness and rostering system experience are essential as you manage client records, service agreements and onboarding while prioritising safety and quality in a busy aged care environment.

Qualifications

• Hands-on experience in Support at Home or CHSP environments.
• Strong knowledge of CHSP/CHIS funding and aged care service requirements.
• Experience in a Care Coordinator, Service Coordinator, Client Services or similar role.
• Strong understanding of care plans, budgets, client records and compliance.
• Excellent communication and stakeholder management skills.
• Ability to manage competing priorities in a fast-paced aged care setting.

We are looking for an experienced Care Coordinator to join our Adelaide team, with strong Support at Home experience and a solid understanding of CHSP/CHIS funding and service delivery requirements.

This is a hands‑on coordination role where you will support clients, coordinate services and work closely with Care Partners, Team Leaders and frontline teams to ensure services are delivered safely, efficiently and in line with funding and compliance requirements.

Key Responsibilities

• Coordinate Support at Home services and ensure clients receive timely, quality care.
• Apply your knowledge of CHSP/CHIS funding, budgets and service requirements to day-to-day coordination.
• Maintain accurate client records, care plans, service agreements and documentation.
• Coordinate client onboarding, service commencement and changes to care arrangements.
