NSW Health in Parramatta is seeking a Chronic & Complex Community Registered Nurse to deliver comprehensive nursing assessment, care coordination and evidence-based clinical interventions for clients with complex needs. You will work autonomously while collaborating with multidisciplinary teams to ensure safe, client-centred care in the community.

The role involves travel across sites and into private residences, with shifts including afternoons and weekends.

Qualificações

• Experience in community health nursing and chronic/complex care.

• Competent in wound, continence and catheter management, palliative care.

Chronic & Complex Community Registered Nurse

This position is required to participate in afternoon, weekend and public holiday shifts.

The rotation to these shifts is minimal at this current time, and is subject to change at anytime.

It is a community health centre based position requiring travel across sites and into private residences.

This position provides care in the community to client's requiring, wound management, continence and catheter management, and supportive and palliative care, including end of life.

What you'll be doing
As a Chronic & Complex Community Registered Nurse, you will provide comprehensive nursing assessment, care coordination and evidence-based clinical interventions to support clients with diverse and often complex healthcare needs.

You will work autonomously while collaborating closely with multidisciplinary teams to deliver safe, effective and client-centred care.

· Conduct holistic nursing assessments and develop tailored care plans

· Coordinate and deliver evidence-based clinical care, monitoring and education

· Partner with clients, carers and families to achieve individual healthcare goals

· Manage complex caseloads while responding proactively to changing clinical needs

· Maintain high-quality documentation and contribute to clinical review and quality improvement activities
