Job summary

Sacred Heart Mission is seeking a casual Support Worker Home Care to assist clients in their own homes across the Northern Metro area, helping them stay independent and engaged in daily life.

You will provide personal care, shopping, transport to appointments, and meaningful social interaction, while completing progress notes and escalating care needs to Coordinators and Case Managers. A Certificate III in Aged Care or similar, a valid driver's license, and relevant experience are required.

Qualifications

• Certificate III in Aged Care or Individual Support (Ageing).
• Current Driver's Licence with reliable registered car and comprehensive motor vehicle insurance.
• Experience in aged care or support services.

Responsibilities

• Help clients within their homes to maintain independence.
• Assist with day-to-day activities and improve quality of life.
• Support clients with personal care, shopping, transport to appointments or a stroll in the park.
• Help clients meet care needs and attend medical appointments.
• Complete progress notes at end of each shift.
• Escalate care needs to Coordinators and Case Managers.
